Why Does Coffee Make You Poop? The Science Explained

Let me just say it: coffee makes a lot of people poop. Research suggests it affects roughly 30-40% of the population this way, and for some of us, the effect is almost immediate, you take a few sips, and within 15-20 minutes, your body sends a very clear signal. It is one of those things everyone experiences but nobody talks about at dinner. Let us talk about it, because the science is genuinely interesting.
It Is Not (Just) the Caffeine
The most common assumption is that caffeine is the culprit. It is a stimulant, so it makes sense that it would stimulate your gut, right? But here is the thing: decaf coffee has a similar effect. Studies have shown that decaf produces nearly the same level of colon stimulation as regular coffee, which means caffeine is, at most, part of the story, not the main driver.
So what else is going on?

Gastrin
When coffee hits your stomach, it triggers the release of a hormone called gastrin. Gastrin tells your stomach to produce more acid (which helps with digestion) and also stimulates motility, the contracting, wave-like movements that push food through your digestive tract. Studies have shown that coffee increases gastrin levels significantly within minutes of drinking, and this effect happens with both caffeinated and decaf coffee. Gastrin is likely the primary reason coffee gets your gut moving.
Cholecystokinin (CCK)
Coffee also stimulates the release of cholecystokinin, a hormone that triggers the gallbladder to release bile and tells the colon to contract. CCK is involved in digestion after meals, but coffee can trigger it independently. This adds another layer to the "coffee makes things move" effect.
The Acidity Factor
Coffee is acidic (pH of about 4.5-5.5, depending on the brew). Stomach acid production increases when you drink something acidic, and increased acid production speeds up stomach emptying. Faster stomach emptying means material reaches your colon sooner. This is another non-caffeine mechanism that contributes to the effect.
Some people find that darker roasts or lower-acid brewing methods (like cold brew) are gentler on their stomach. If coffee makes your gut uncomfortably active, switching to cold brew or a darker roast might reduce the intensity without eliminating it entirely.
The Cream and Milk Factor
Here is a twist that many people do not consider: if you add milk or cream to your coffee and are even mildly lactose intolerant (which affects an estimated 68% of the global population to some degree), the dairy can amplify the digestive effect significantly. The combination of coffee's natural motility stimulation plus lactose intolerance can create a very urgent situation.
If you drink coffee with milk and experience strong digestive effects, try drinking it black for a week and see if the urgency decreases. If it does, the dairy is a significant contributor.
Does It Happen to Everyone?
No. Studies show that about 30-40% of people experience a noticeable laxative effect from coffee. The other 60-70% either have a milder response or no response at all. Genetics, gut microbiome composition, and individual sensitivity to gastrin and other hormones all play a role. If coffee does not send you to the bathroom, you are in the majority, the rest of us just talk about it more.
Is It Bad for You?
For most people, no. Coffee stimulating your colon is a normal digestive response, not a sign of damage or irritation. If anything, gentle stimulation of gut motility is considered healthy, constipation is a far bigger health concern than regular bowel movements.
However, if coffee causes you genuine discomfort, cramping, diarrhea, or urgency so strong it disrupts your routine, it is worth paying attention to:
- Rule out dairy issues first (try black coffee).
- Try a lower-acid coffee (cold brew, dark roast, or low-acid blends).
- Do not drink coffee on a completely empty stomach. Eating something first, even a small snack, can buffer the gastric response.
- Talk to your doctor if the effects are severe or accompanied by pain. Coffee can worsen existing conditions like IBS or gastroesophageal reflux (GERD).
